When your decision has been made, fill out an "A" requisition (form 60-005-803), including your cost center,
authorized signature with the list of the equipment and accessories you're ordering. If you are replacing a current cell phone put in the body of the
"A" requisition that this is a replacement phone and list the cellular number. Fax the purchase requisition to (615) 343-1605, campus mail it to ITS,
P.O. Box 34, Peabody or bring it to our office at 143 Student Hill Center addition.
When ITS receives the requisition, we will fax it to Sprint to process the order. When Sprint receives the requisition
they will order the equipment and it will be delivered to the shipping address (UPS) specified within 3 to 5 working days. Note: Once the phone has been
delivered, there is a 30-day return policy. Departments can arrange bulk orders with Justin Gabbard directly for Vanderbilt-paid business use phones.
